Eyelid lift surgery (Part 1)

DR. NASSIF: Let's take a look at the manikin here. I love this skin here. (Isn't that cool?)
In regard to the brow lift, I am going to point real quick right over here. We have one little incision right there, one incision right there, one incision in the center and two on the other side. That is for the brow lift, which is lifting up the eyebrow. With this technique we are able to use the endoscope through small incisions. In general when we take a look right over here, I am going to kind of lift the skin up here and we are going to gentle and naturally lift up the eyebrow. Part two of this, there is a little crease, a natural crease right here. We want to make sure that we make that incision in the crease so that we can hide it. At that point we can still see if there is a little extra skin on the upper eye. If there is, that is when the upper eyelid lift or upper blepharoplasty is necessary.
So brow lift first in many scenarios followed by an upper eyelid lift.
DR. NASSIF: You have to remember the Ultratine takes about nine or ten months before it completely dissolves and goes away. Then you will feel nothing.
HOST: Now let's look at what Guy did and looking at his pictures I know this was not a chip shot.
DR. MASSRY: His lower lids were complex. The lower lids in general are a complex structure. There are multiple layers. The problem with Bob's face is he did not want the shape of his eyes changed and any standard blepharoplasty I used would have changed the shape of your eyes. If you look at yourself right now, you have perfect almond shaped eyes, they are smooth. There are festoons, the little malar bag that Bob and I discussed beforehand with his family.
BOB: These pouches here, that is what I originally talked about doing something with and that is how the whole process started. Then they explained to me in detail what was possible and what was not possible.
DR. MASSRY: What I tell people with malar bags or festoons is if you really do not like them and you want to aggressively go after them, there is a chance that it will change the shape of your lid. If you are willing to accept a 30%, 40% or 50% improvement to look natural and note know that anything was done, that is fantastic. The one thing I will say is if you look at Bob right now, I don't think anyone can tell any incision was made. It was a little tough with the skin but during this surgery this whole layer of skin was elevated, the muscle layer underneath was elevated and secured to the bone up here. The cartilage of the lid was tightened and some skin was taken out. What I told Bob was we are going to get you 25% to 30% better and I don't know how you judge it right now.
BOB: I think you got 70%.
HOST: You essentially did a mid facelift and had to do an eyelid suspension.
DR. MASSRY: I think the real critical thing over here and you said it before is when you are going to have a procedure done, it is the consultation, it is your expectations and it is what is told to you beforehand.
