Female patient with post-operative nasal obstruction and unhappy with the appearance of her nose following a two previous rhinoplasties by another surgeon.

Procedure: Revision Rhinoplasty with ear cartilage for reconstruction of external valve collapse (over-resected tip cartilage) with lateral crural strut grafts, shield graft and osteotomies for an inverted V deformity.

Comment: After 5 months, her nose appears softer and natural with relief of her nasal obstruction. Her main desire was to have her "old, ethnic nose back". Patients that undergo "Westernization Rhinoplasty" to achieve a smaller nose are more at risk for tip collapse following aggressive removal of tip cartilage since they have thick skin. The thick skin needs strong cartilage support to achieve a thinner, more caucasian appearance.
