Female patient with nasal obstruction and an external nasal deformity. Patient’s chin is also set back and desires augmentation.
PROCEDURE: Septoplasty, turbinoplasty, open rhinoplasty with placement of bilateral spreader grafts, columellar strut, hump reduction, tip cartilage reduction with suturing techniques, medial crural overlay to decrease tip projection and lateral crural strut grafts to prevent alar collapse and platelet gel application.
OTHER PROCEDURE: Chin Implant with small pre-jowl/chin implant.
FINDINGS: Septum deviated to left with caudal deviation to right, bilateral narrow middle vault with left greater than right, convex lower lateral cartilages (LLC), left greater than right alar retraction, left LLC retrodisplaced as compared to right with over-projection (bilateral) LLC.
COMMENT: The 4 and 7 month photos reveal a deprojected softer profile. The tip and middle vault is still healing and is slightly swollen. The base view reveals an open airway. Also note that the deprojected tip makes the base view appear wider. |
