Patient Experience
Preoperative Diagnosis: Nasal obstruction secondary to external/internal valve collapse, deviated septum, hypertrophic turbinates. Submental & jowl lipodystrophy, platysmal banding, and microgenia.
Procedure: Septoplasty, Bilateral Turbinoplasty, Septorhinoplasty, Chin implant
Findings: Left > right external valve weakness, rim weakness, 3+ middle vault weakness/narrowing; septum with C-shaped deformity with anterior septum convex to right; 3+ spur at base of left airway on septum; septal-crural ligament lysed to deproject; twisted nasal bones; vertically-oriented LLC (lower lateral cartilages) with left LLC cephalad to Right LLC; left wide dome; asymmetric dome, left LLC concave. Boxy, splayed and bifid tip.
Comments: This patient desired an improvement in his nose and a more masculine chin. Post surgery photos reveal an improved, stronger profile.

“Since I was ten years old, my nose was broken playing baseball a baseball bat hit it. One of the kids swung the bat, hit the ball, and then threw the bat and hit me in the nose. But it healed and I was ok but as I’ve grown it started to deviate my septum a little bit to the side so I’ve always breathed through my mouth.

Concepts Related to Jeff’s Procedure
Septoplasty: Septoplasty is a surgical procedure used correct the shape of the septum due to defects or deformities. The nasal septum is the separation between the nostrils. Septoplasty may be performed during a rhinoplasty procedure. When performed together, this is referred to septorhinoplasty.
Turbinoplasty: There are three turbinates in each nasal pasage. Each one protrudes from the sidewall of the nose. The lower or inferior turbinate is by far the largest of the three. Turbinates are shaped like a scroll, and act to warm, air before it passes on to your throat. Throughout the day, the turbinates periodicallychange size in a pattern known as the nasal cycle. That’s why at any given time, a person can usually breathe better out of one side the other. Turbino plasty is the operation performed to reduce the size of the turbinate. Usually, only the inferior turbinates need correction.
