Patient Profile: Erika

Preoperative Diagnosis: Septal deviation, turbinate hypertrophy, nasal obstruction, external nasal deformity.

Procedure: Ethnic septorhinoplasty, bilateral turbinoplasty, left ear graft conchal harvest (see photo), right deep temporalis
fascia harvest.

Findings: Deviated nasal septum, 3+ thick skin/ nasal SMAS, weak, deprojected, flat lower lateral cartilages, deep radix and
low dosum. Nose was poorly projected/over-rotated. Following surgery, increased projection, counter-rotated with nasolabial angle opened.

Comments: Her early post-op photos show an improved nose. She had alar base reduction which is healing nicely. The back of the ear photo reveals the closed incision following ear cartilage harvest.

Question:
I need to have my nose fixed. I had rhinoplasty several months ago. What a disaster! Now I have a "pig" nose.  It is up turned so much you can see inside my nostrils. I cannot believe this happened. I am so depressed about it. Please, please tell me that this is repairable. Is recovery from revision rhinoplasty about the same as the original surgery?

Answer:

This is definitely something that may be repaired. When performed properly by a highly skilled revision rhinoplasty surgeon, revision rhinoplasty may yield excellent results. The recovery from revision rhinoplasty will not be much different than the recovery from the primary rhinoplasty. However, the swelling may last longer following the revision rhinoplasty. Question:
I am African American and would like to undergo rhinoplasty. I know that I want my nostrils narrowed and the tip fixed but I am not exactly sure of the final shape. Do you have an imaging computer program that will allow me to see my face with different noses?

Answer:

Yes. In my office I have a computer morphing program that allows the patient to get an idea of what the outcome of their surgery may look like. It gives a pretty good idea of the final results. The goal of the program is to provide me with a guideline that I follow during surgery. During a consultation I will communicate with the patient and create an image that looks natural and follows what they would like to achieve with their procedure. Thank you for the question.

Question:
I had rhinoplasty 3 weeks ago. When the doctor removed my bandage I noticed that my pores were extremely clogged and the skin around my nose is peeling. My nose is still sensitive to touch and I am afraid squeeze it. What happened and Is there anything I can do about this?

Answer:

Don't panic!  There are a variety of things that can be done to restore your skin to normal.  Your bandage was most likely on for for a week or two, during this time excess oil, skin, and dirt built up under the bandage causing blackheads to form.  Your skin may be peeling due to the change in sleep and diet patterns or the healing that is taking place in and around the surgical area.At home you can start by using a good cleanser with water and a soft washcloth.  A gentle... continue
